Paper Applications

Paper applications allow you to collect information about applicants and their families. They are generally completed by a parent/guardian or an agency representative who is assisting the family with the application process. In order to improve data entry efficiency, ChildPlus Software has specifically designed these paper application forms to follow the flow and order of the system.

Paper application forms designed specifically for ChildPlus are available in both English and Spanish. While both PDF and Word versions of our applications are available, we only recommend downloading the Word versions if you plan to customize the application forms for your agency.

Printing Paper Applications from Within ChildPlus

Open the Application module and select the participant whose application you want to print.
Click Print Application.
Check each applicable box to indicate the application form(s) you want to print.

Check This Box	To Print
Print the Family Information page	The information you recorded in the Family Information section of the application.
Print the Family Member page(s)	Basic information for each member of the family.
Print Eligibility Verification Page	The participant's eligibility verification information.
Family Members who are applying for enrollment	Detailed information about each family member you select.
Enrollment records you want to print	Enrollment information for each family member you select.

Click Preview to view the application forms on the screen or click Print to print out a copy of the participant's application.
